Certified copy of child's birth certificate
Immunization (shot) records
Withdrawl Form from previous school
Report Card from previous school
Proofs of residency, such as:
ONE OF THE FOLLOWING:
Rental/Mortgage Agreement
Lease Agreement
Electric/Gas Bill
Water Bill
Phone Bill
Credit Card Bill
Bank Statement
Car Insurance
If you live outside of your school boundaries or district boundaries, and would like to attend a Pendergast School. Please fill out the Open Enrollment form.
