    Enrollment Packet

    You can complete this packet and take it with you when you register your child at the Pendergast Learning Center. Enrollment Packet: English | Español

    What you need to register:

    1. Certified copy of child's birth certificate
    2. Immunization (shot) records
    3. Withdrawal Form from previous school
    4. Report Card from previous school
    5. Proofs of residency, such as:

    One of the following:

    • Rental/Mortgage Agreement
    • Lease Agreement
    • Electric/Gas Bill
    • Water Bill
    • Phone Bill
    • Credit Card Bill
    • Bank Statement
    • Car Insurance

    If you live outside of your school boundaries or district boundaries, and would like to attend a Pendergast School. Please fill out the Open Enrollment form.
